Research exposes common profiles that existed in schools, workplaces, and communities who failed to prevent incidents and tragedies that post-event reports revealed were preventable. What were the common gaps, silos, and disconnects with  failed preventions for the last 20+ years?

Most (if not all) of the failed preventions had more than enough pre-incident indicators that were exhibited by at-risk individuals and observed by others in the schools, and/or workplace, and/or community and/or online.
